Attendance Exception Request

FIRST THINGS FIRST - read the Handbook (click here) and Grading Policy within the Handbook - this will help guide you in making decisions regarding absences from band events.

Next - make sure you are familiar with approval/disapproval scenarios (click here). This document was created by band students during the 2010-2011 school year and was adopted as policy for all future years.

READ EVERYTHING - this will prevent mistakes and hurt feelings if you are not approved and lose grade points.

This form is to be used when a student needs an exception to the attendance policy and only in extreme/special circumstances.
A student should not assume this form will be automatically approved/disapproved and should wait for notification before making further arrangements.
Please note that the calendar appears every year in March/April for the upcoming school year and exceptions to previously published performance dates will be very rare.
Athletic/Activity absences must be accompanied by your complete sport/activity calendar. These will only be approved for LHS sponsored sports/organizations.

This form cannot be used for a No-Call/No-Show situation.
